A Personal Health Record is typically a health record that is initiated and maintained by an individual. An ideal personal health record system would provide a complete and accurate summary of the health and medical history of an individual by gathering data from many sources and making this information accessible online to anyone who has the necessary electronic credentials to view.
Personal Health Records can contain a diverse range of data but usually include information about:
- blood type
- allergies and adverse drug reactions
- medications (including dose and how often taken) including over the counter medications and herbal remedies
- illnesses and hospitalizations
- surgeries and other procedures
- laboratory test results
- family history
Maintaining a personal health record system is essential to having access to your health information over the course of your lifetime and lets you or your family, physicians, and emergency personnel access vital information as stated above and much more. By providing critical information at your discretion, within seconds, a personal health record system can save time and your life!
